To Choose A Collection Site Press The Blue Bar.

Then Press The Blue 'Make Appointment' Button. Note - clear the popups on Labcorp.com site then scroll down to see locations and make appointment.
Click DONE (Bottom Left) When Finished with LabCorp.

Call the Collection Site *Before You Go* to make sure they are open.

Click the Top HOME BAR To Go to Collector Mobile Authorization Form

Show the Collector your Mobile Authorization by clicking On Mobile Authorization Form on the Front Page Link of This App.

Write down the time and location of your appointment or take a screen shot so you do not forget.
